Coronary Heart Disease

Question. Describe etiology, clinical features and management of coronary artery disease.

Answer. Coronary artery diseases are also known as ischaemic heart disease.

The following are the coronary artery diseases:

  1. Angina pectoris
  2. Acute myocardial infarction
  3. Sudden cardiac death.

Aetiology Of Coronary Artery Diseases

1. Atherosclerotic coronary artery disease

2. Other coronary artery diseases:

  1. Coronary artery spasm
  2. Coronary arteritis
  3. Embolism
  4. Coronary AV malformation

3. Valvular diseases:

  1. Aortic stenosis and regurgitation
  2. Mitral valve prolapse

4. Other cardiac diseases:

  1. Hypertrophic cardiomyopathy
  2. Collagen disease
  3. Syphilis

5. Increased demands:

  1. Thyrotoxicosis
  2. Anemia
  3. Beriberi

Clinical Features Coronary Artery Disease

  1. Angina pectoris: Refer to Ans 22 of same chapter.
  2. Acute myocardial infarction: Refer to Ans 21 of same chapter
  3. Sudden cardiac arrest: In this, a person with good health fall ill and die suddenly within minutes and a few hours.
    1. Absence of pulses
    2. On auscultation, cardiac impulse is not present.
    3. Extremities are cold
    4. Cessation of respiration of the patient
    5. Blood pressure is not measurable.
